If you've been told "it might be SIBO," the next question is almost always: okay, so how do I actually find out? The answer isn't a single test — it's a handful of options that measure different things, in different ways, with different tradeoffs. None of them is perfect, and knowing the pros and cons of each can make the difference between a confusing result and a useful one.

The Gold Standard: Small Bowel Aspirate and Culture

This is the most direct way to test for SIBO — during an upper endoscopy, a physician draws fluid directly from the small intestine (usually the duodenum or jejunum) and sends it to a lab to be cultured and counted. A bacterial count of 10⁵ colony-forming units per milliliter or higher is considered a positive diagnosis.

Pros: It's about as close to definitive as testing gets, with sensitivity and specificity approaching 100%. There's no guessing about what the numbers mean.

Cons: It requires an endoscopy — sedation, cost, scheduling, and some procedural risk — which makes it impractical as a first-line or repeatable test. It also only samples one point in the small intestine, so overgrowth further along can be missed.

Because of the invasiveness, this test is used far less often than breath testing — even though it's the more accurate of the two.

For most people, this isn't the starting point. It's the backup option, or the next step when a breath test result and the symptom picture don't line up.

Breath Testing: The Practical Default

Breath tests work on a simple principle. Bacteria (or archaea, in methane's case) produce gas when they ferment carbohydrates. Human cells don't produce hydrogen or methane at all, so any of it showing up in your breath came from gut microbes — and if it shows up fast, that suggests those microbes are further up the digestive tract than they should be, in the small intestine rather than the colon.

You drink a sugar solution, then breathe into collection tubes at set intervals (typically every 15–20 minutes for 2–3 hours), and a lab measures the gas levels in each sample. Two separate choices shape what the test can tell you: which sugar you drink, and which gases get measured.

Choice One: Glucose or Lactulose

Glucose is absorbed quickly, almost entirely within the first few feet of the small intestine. That makes it a more targeted test — if it detects a gas spike, that fermentation is very likely happening in the upper small intestine, since little to no glucose survives to reach the colon.

  • Pros: Higher specificity, meaning fewer false positives. Many people also find the taste easier to tolerate.
  • Cons: Because it's absorbed so early, glucose may never reach bacteria overgrowing further down the small intestine — leading to false negatives in distal SIBO. It also involves a real sugar load (around 75g), which can be a consideration for anyone managing blood sugar.

Lactulose is a synthetic sugar the body can't digest or absorb at all, so it travels the entire length of the small intestine before reaching the colon.

  • Pros: Better sensitivity across the full small intestine, including distal overgrowth that glucose might miss.
  • Cons: Higher rate of false positives, partly because once lactulose reaches the colon (which it always eventually does), the resulting gas spike can be mistaken for small intestinal fermentation rather than normal colonic activity.

A meta-analysis found glucose testing to be more specific (fewer false positives), while lactulose is more sensitive (fewer false negatives) across a wider intestinal range. Neither sugar is universally "better" — which one makes sense depends on where your symptoms and history point.

Choice Two: Which Gases Get Measured

Hydrogen is the classic target, and it's what most basic SIBO breath tests measure. Elevated hydrogen is generally associated with a diarrhea-predominant presentation.

Methane is produced not by bacteria but by archaea — a technical distinction that led to a recent renaming: excess methane is now more accurately called Intestinal Methanogen Overgrowth (IMO) rather than "methane-dominant SIBO." Methanogens tend to live lower in the gut and slow intestinal transit down, which is why elevated methane correlates strongly with constipation and constipation-predominant IBS.

Hydrogen sulfide is the newest addition, available through specific tests like trio-smart. This matters because some people have normal hydrogen and methane levels but are still symptomatic — and elevated hydrogen sulfide can be the missing piece, associated with its own pattern of symptoms (often diarrhea and visceral sensitivity). Standard two-gas breath tests simply can't see it, so it can lead to a falsely reassuring "normal" result.

A combined hydrogen + methane + hydrogen sulfide test gives the fullest picture, though it's not always the most accessible or lowest-cost option depending on where you're tested.

What Makes Any Breath Test Less Than Perfect

Even the right substrate-and-gas combination has real limitations worth knowing before you test.

False positives can come from eating fermentable, high-FODMAP foods too close to the test, from testing beyond the 60–90 minute diagnostic window (gas from normal colonic fermentation starts to show up and gets misread as small intestinal), or from rapid gut transit sending the substrate to the colon faster than expected.

False negatives happen in an estimated 20–30% of true SIBO cases — often from recent antibiotic use, slow transit delaying the gas response past the test window, or a hydrogen sulfide overgrowth that a two-gas test can't detect at all.

Preparation matters a lot, and skipping or shortcutting it is one of the most common reasons for a misleading result. Most protocols call for:

  1. Avoiding antibiotics for several weeks prior
  2. Avoiding gas-producing and high-FODMAP foods for 24–72 hours before
  3. A straightforward low-fermentation diet the day before
  4. A genuine 8–12 hour fast going into the test

None of this means breath testing isn't useful — it's the most practical, accessible, and repeatable option most people have. It just means a single result, especially a borderline one, is a data point to discuss with a provider rather than a final verdict on its own.

So Which Test Should You Actually Get?

For most people, a hydrogen and methane breath test (glucose or lactulose, depending on symptom pattern) is a reasonable starting point — noninvasive, widely available, and informative enough to guide next steps. If symptoms persist despite a negative result, a hydrogen sulfide-inclusive test or a conversation about endoscopic aspirate testing may be the more useful next move. This is a decision worth making with a provider who can weigh your specific symptom pattern (diarrhea-leaning versus constipation-leaning is a meaningful clue) alongside the test's known blind spots.

Whatever test you land on, the result is more useful in context. Tracking your symptoms consistently before and after testing — what's flaring, what's not, and when — gives you and your provider something concrete to compare the test result against, rather than relying on memory alone.
