The Short Answer
SIBO stands for Small Intestinal Bacterial Overgrowth. It happens when too many bacteria build up in the small intestine — the long tube between your stomach and your colon where most of your digestion and nutrient absorption happens.
Your small intestine is supposed to have very few bacteria compared to your colon. When that balance gets thrown off and bacteria move in (or multiply where they shouldn't), those bacteria start feeding on the food you eat before your body gets the chance to. That fermentation process produces gas, and that gas is what causes many of the classic SIBO symptoms: bloating, pain, and irregular bowel movements.
SIBO isn't really about "bad bacteria." It's about bacteria — even normal, everyday bacteria — ending up in the wrong place, in the wrong amount.
A Healthy Small Intestine, Explained
To understand what goes wrong with SIBO, it helps to know what's supposed to happen.
Your gut isn't evenly populated with bacteria from top to bottom. Your colon (large intestine) is home to trillions of bacteria — that's normal and healthy. Your small intestine, on the other hand, is supposed to stay relatively clear. Several systems work together to keep it that way:
- Stomach acid kills off most bacteria that come in with food, before they ever reach the small intestine
- Bile and digestive enzymes further break down food and discourage bacterial growth
- Peristalsis — the rhythmic muscle contractions that move food through your gut — keeps things moving so bacteria don't have time to settle in and multiply
- The Migrating Motor Complex (MMC) is a special "cleaning wave" that sweeps through the small intestine between meals, clearing out leftover food particles and bacteria
- The ileocecal valve, a one-way valve between the small intestine and colon, normally stays shut to stop colon bacteria from creeping backward into the small intestine
- A healthy mucus lining and immune activity in the gut wall add another layer of defense
When all of these systems are working, your small intestine stays lightly populated with bacteria, and digestion runs smoothly. SIBO happens when one or more of these defenses breaks down.
So What Actually Goes Wrong?
SIBO can develop a few different ways, but they usually come down to one of these:
- Something slows things down. If food and waste move through the small intestine too slowly, bacteria have more time to sit, feed, and multiply.
- A physical barrier fails. If the ileocecal valve doesn't close properly, or scar tissue (adhesions) from past surgery changes the shape of the intestine, bacteria can end up somewhere they shouldn't be.
- The defenses themselves weaken. Long-term use of acid-reducing medication, for example, removes one of the body's natural barriers against bacterial overgrowth.
Once bacteria establish themselves in the small intestine, they start fermenting carbohydrates from your food, producing hydrogen gas, methane gas, or both. That fermentation is the direct cause of most SIBO symptoms — and it's also what breath tests are designed to detect.
How Common Is SIBO?
This is a harder question to answer precisely than you might expect. Estimates in the research range widely — anywhere from about 2.5% to 22% of the general population, and 6–15% of people with no digestive symptoms at all may still test positive. The wide range exists because SIBO symptoms overlap so heavily with other conditions, and because breath testing itself has real limitations (more on that below).
What's clearer is who's at higher risk:
- Older adults — prevalence climbs to around 14–16% in elderly populations, likely because motility naturally slows with age
- People with diabetes — SIBO shows up in an estimated 29% of people with diabetes, largely due to nerve damage that affects gut motility
- People with other digestive or motility disorders, including IBS, inflammatory bowel disease, and connective tissue disorders like scleroderma
- People who've had abdominal surgery, which can create scar tissue or alter gut anatomy
What Causes SIBO?
There's rarely one single cause — SIBO is usually the downstream result of something else going on in the body. The most commonly identified contributors include:
- Post-infectious motility damage. A bout of food poisoning can trigger the immune system to produce antibodies that mistakenly damage the nerves controlling the MMC. This single mechanism is thought to explain a large share of SIBO cases.
- Slow gut motility (dysmotility) from any cause, including diabetes, hypothyroidism, or connective tissue disorders
- Structural changes, such as adhesions from prior surgery, or anatomical differences that create pockets where digestive flow slows down
- A weakened ileocecal valve, allowing colon bacteria to migrate backward
- Long-term use of certain medications, particularly proton pump inhibitors (which reduce stomach acid) and opioids (which slow motility)
- Chronic conditions like inflammatory bowel disease, which change gut structure and immune activity over time
Because SIBO is so often a symptom of an underlying issue rather than a standalone problem, understanding the "why" behind an individual case is a meaningful part of treating it — and preventing it from coming back.
What Does SIBO Actually Feel Like?
The most familiar symptoms are digestive:
- Bloating, often worse as the day goes on
- Gas and belching, especially after meals
- Abdominal pain or cramping
- Diarrhea, constipation, or a pattern that alternates between the two
- Feeling full after eating only a small amount
- Greasy, floating, or foul-smelling stools
But SIBO doesn't always stay confined to the gut. When it goes on for a long time, it can interfere with how well your body absorbs nutrients — and that can quietly show up as fatigue, brain fog, hair thinning, skin changes like rosacea or dryness, joint aches, and more. These symptoms are easy to miss or attribute to something else entirely, which is part of why SIBO often goes undiagnosed for months or years.

SIBO vs. IBS — What's the Difference?
This is one of the most common points of confusion, and for good reason: the two conditions overlap a lot.
IBS (Irritable Bowel Syndrome) is a functional disorder — meaning it's diagnosed based on a pattern of symptoms, not a specific physical test. It reflects how the gut and brain communicate, including heightened sensitivity to normal digestive processes.
SIBO is more of a physical, measurable problem — an actual overgrowth of bacteria that can (in theory) be tested for directly.
Here's where it gets complicated: research suggests a substantial share of people with IBS — some studies estimate around a third or more — also test positive for SIBO. SIBO may be an underlying driver of IBS symptoms for some people, while for others, the two conditions coexist without one necessarily causing the other. This is also why treating SIBO doesn't always fully resolve IBS symptoms — the gut-brain sensitivity behind IBS can persist even after bacterial overgrowth is addressed.
How Is SIBO Diagnosed?
The most common approach is a breath test. You drink a sugar solution (either glucose or lactulose), then breathe into collection tubes over a couple of hours. Because human cells don't produce hydrogen or methane gas, any of it in your breath came from bacteria fermenting the sugar — and how quickly it shows up gives clues about where in your gut that fermentation is happening.
The most accurate method is a small bowel aspirate and culture, done during an endoscopy, where fluid is drawn directly from the small intestine and tested in a lab. It's far more precise, but it's invasive, so it's typically reserved for cases where breath testing hasn't given a clear answer.
Breath testing isn't perfect — false positives and false negatives both happen, often due to test preparation, gut transit speed, or which gases were actually measured (a standard two-gas test can miss hydrogen sulfide-driven cases entirely).

How Is SIBO Treated?
There's no single protocol that works for everyone, but effective treatment generally moves through three stages:
- Reduce — lowering the bacterial overgrowth itself, typically with a prescription antibiotic like rifaximin, sometimes combined with a second antibiotic for methane-dominant cases, or in some cases an herbal antimicrobial protocol
- Restore — supporting normal gut motility (especially the MMC) so bacteria don't simply repopulate, often through prokinetic medications, meal spacing, and lifestyle factors like sleep and stress management
- Rebuild — gradually expanding the diet, correcting any nutrient deficiencies, and rebuilding a comfortable, sustainable relationship with food after a period of restriction
Treating the bacterial overgrowth without addressing why it happened in the first place is one of the most common reasons SIBO comes back. Long-term success usually depends on identifying and managing the underlying cause, not just clearing the current overgrowth.

What Happens If SIBO Goes Untreated?
Most people with SIBO are dealing with uncomfortable, disruptive symptoms rather than a dangerous condition — but leaving it unaddressed for a long time does carry real risks, largely related to ongoing nutrient malabsorption:
- Vitamin and mineral deficiencies, particularly B12, iron, and the fat-soluble vitamins (A, D, E, K)
- Unintended weight loss and malnutrition in more severe or long-standing cases
- Bone density loss over time, linked to poor calcium and vitamin D absorption
- Nerve-related symptoms, such as tingling or numbness, in cases of significant B12 deficiency
This is why getting an accurate diagnosis and a plan — rather than guessing indefinitely — matters, even when symptoms feel manageable day to day.
Is SIBO Curable?
This is one of the most honest, and most important, things to understand about SIBO: for many people, it's manageable rather than something with a single, permanent fix.
Some people clear SIBO with one round of treatment and don't have it return. Others find it comes back and requires ongoing attention — particularly if the underlying cause (like slow motility or a structural issue) hasn't fully resolved. That's not a personal failure or a sign that treatment "didn't work." It reflects how complex and individual this condition actually is.
The most realistic goal isn't necessarily a permanent cure — it's a manageable, well-understood condition with a plan you can return to when symptoms flare, and a clear sense of what tends to help your body specifically.
When to Talk to a Doctor
Persistent bloating, abdominal pain, unexplained changes in bowel habits, or symptoms that interfere with daily life are all good reasons to bring SIBO up with a healthcare provider — especially if basic bloodwork hasn't ruled out nutrient deficiencies.
