What Is SIBO?

SIBO stands for Small Intestinal Bacterial Overgrowth. Your gut is full of bacteria — trillions of them — but they're supposed to live mostly in your large intestine (colon), where they help digest fiber and produce useful byproducts. Your small intestine, by contrast, is meant to stay relatively low in bacteria, because that's where most of your nutrient absorption happens.

SIBO occurs when bacteria that belong in the colon migrate upstream and take up residence in the small intestine — or when the bacteria that are supposed to be there multiply out of control. Once they're there, they start doing what bacteria do: fermenting the food you eat before your body has a chance to absorb it. That fermentation produces gas — mostly hydrogen, methane, or in some cases hydrogen sulfide — and that gas is where most of the classic SIBO symptoms come from.

It's worth saying clearly: SIBO is not a disease in itself. It's a downstream consequence of something else going wrong. That "something else" is usually a breakdown in one of the systems that normally keeps bacteria in check — and understanding that is the key to actually getting better, not just treating symptoms on repeat.

What normally prevents SIBO

A few systems work together to keep your small intestine relatively bacteria-free:

  • The migrating motor complex (MMC) — a wave of muscular contractions that sweeps through your small intestine between meals, acting like a housekeeping crew that clears out leftover food and bacteria. When this doesn't work properly (often after food poisoning, chronic stress, or certain medications), bacteria can accumulate.
  • Stomach acid — a strong acidic environment kills off much of the bacteria you swallow before it reaches your small intestine. Long-term use of acid-suppressing medications can reduce this defense.
  • The ileocecal valve — a one-way valve between the small and large intestine that's supposed to prevent colonic bacteria from backing up into the small intestine.
  • Bile and pancreatic enzymes — these have antimicrobial properties in addition to their digestive role. When one or more of these breaks down, bacteria get a foothold, and SIBO can develop.

Common Symptoms

SIBO symptoms vary from person to person, which is part of why it's so often missed or misdiagnosed. That said, a few patterns show up consistently:

Digestive symptoms

  • Bloating, especially that worsens as the day goes on

  • Abdominal pain or cramping

  • Excessive gas

  • Diarrhea, constipation, or an alternating pattern of both

  • A feeling of fullness after eating small amounts

  • Fatigue or brain fog

  • Nutrient deficiencies (especially B12, iron, and fat-soluble vitamins)

  • Unintentional weight loss or, in some cases, weight gain

  • Joint pain One of the clearest signals people describe is a bloating pattern that's flat in the morning and progressively worse by evening, often looking noticeably different by dinnertime than it did at breakfast — a pattern that's less typical of simple food intolerance and more suggestive of bacterial fermentation building up over the day.

Why SIBO Gets Missed

Several things make SIBO tricky to catch:

  1. Symptom overlap with IBS. In fact, research suggests a meaningful subset of people diagnosed with IBS actually have underlying SIBO. The symptoms look identical from the outside, so without specific testing, it's easy to land on an IBS label and stop looking further.
  2. Normal standard labs. Routine bloodwork doesn't screen for SIBO. You can have textbook-normal labs and still have significant bacterial overgrowth.
  3. It's not something most doctors think to test for. SIBO testing isn't part of a standard workup unless a provider specifically suspects it, so many people go years without ever having it mentioned.
  4. Root causes are often overlooked. Even when SIBO is diagnosed and treated, if the underlying cause (motility issues, prior food poisoning, structural issues, medication effects) isn't addressed, it tends to come back.

How SIBO Is Diagnosed

Breath testing

The most common and accessible way to test for SIBO is a lactulose or glucose breath test. Here's how it works, in plain terms:

You drink a sugar solution (lactulose or glucose), then breathe into a collection tube at set intervals — typically every 15–20 minutes over 2–3 hours. If bacteria are present in your small intestine, they'll ferment that sugar and produce gas, which shows up in your breath as hydrogen and/or methane. The pattern and timing of the rise in these gases helps determine whether — and what type of — overgrowth is present.

There are three general subtypes worth knowing:

  • Hydrogen-dominant SIBO — often associated with diarrhea-predominant symptoms
  • Methane-dominant SIBO (sometimes now called IMO — intestinal methanogen overgrowth) — often associated with constipation
  • Hydrogen sulfide SIBO — a newer subtype linked to diarrhea and sometimes a distinct rotten-egg-smelling gas; testing for this is less widely available but becoming more common Breath testing isn't perfect. False positives and false negatives happen, timing and prep matter a lot, and results should ideally be interpreted alongside your symptom picture — not in isolation.

Other tools your provider might use

  • Comprehensive stool testing — doesn't diagnose SIBO directly but can offer clues about digestion, inflammation, and gut ecosystem balance
  • Organic acids testing — sometimes used to look for bacterial or fungal byproducts
  • Endoscopy with small intestine aspirate — considered a gold standard but rarely used in practice due to its invasiveness and cost

Preparing for a breath test

Most protocols require a specific preparatory diet the day before (typically low-fermentable foods) and a fasting window the morning of. Certain medications and supplements — antibiotics, probiotics, and some prokinetics among them — usually need to be paused beforehand. Because prep requirements vary by lab, it's worth following the specific instructions your test provider gives you rather than generic guidance.


Treatment Options

There's no single "right" way to treat SIBO — the right approach depends on your subtype, symptom severity, root cause, and personal preferences. Here's the landscape of what's generally available, so you can have an informed conversation with your provider.

1. Antimicrobial treatment (clearing the overgrowth)

This is usually the first phase of treatment, and it comes in two main flavors:

  • Prescription antibiotics — Rifaximin is the most studied and commonly used, particularly for hydrogen-dominant SIBO, in part because it acts locally in the gut rather than being absorbed systemically. Methane-dominant cases are often treated with a combination approach.
  • Herbal antimicrobials — Combinations of herbs like berberine, oregano oil, neem, and allicin have research support and are a common alternative or complement to antibiotics, often used over a longer course. Neither route is universally "better" — they're different tools, and your practitioner can help you decide based on your subtype, history, and preferences. This is a conversation to have with a doctor or qualified practitioner rather than a decision to make from a guide alone, especially since dosing and duration are individualized.

2. Diet as a management tool (not a cure)

Diet can meaningfully reduce symptoms during treatment by limiting the fuel available to bacteria, but it's generally considered a management strategy, not a standalone fix. Common approaches include:

  • Low FODMAP diet — reduces fermentable carbohydrates that feed bacterial overgrowth
  • SIBO-specific diets (like the SIBO Bi-Phasic diet or SIBO Specific Food Guide) — more targeted variations built specifically around SIBO
  • Elemental diet — a liquid, pre-digested formula used short-term in more severe cases, designed to "starve" bacteria while still nourishing you A word of caution: restrictive diets can bring fast relief, but staying on them long-term isn't the goal. 3. Addressing the root cause

This is the step that's most often skipped — and the reason SIBO recurs in a large percentage of cases. Some of the most common root causes include:

  • Impaired motility (MMC dysfunction) — often addressed with a prokinetic (prescription or herbal) taken after treatment to help keep bacteria from re-accumulating
  • Low stomach acid — sometimes related to long-term use of acid-reducing medications
  • Structural issues — adhesions from prior surgery, diverticula, or an impaired ileocecal valve
  • Prior food poisoning — a well-documented trigger, since certain toxins from food poisoning can damage the nerves that drive the MMC

4. Supporting the gut afterward

What a Realistic Timeline Looks Like

There's no universal timeline, but a general shape many people experience:

  1. Symptom recognition — weeks to years, unfortunately, before SIBO is even considered
  2. Testing and diagnosis — a breath test and results, typically within a couple of weeks
  3. Initial antimicrobial treatment — commonly 2–6 weeks, depending on the approach
  4. Symptom improvement — some people notice change within days of starting treatment; others take the full course to see meaningful shifts
  5.
